Transcript Request
Horizon College & Seminary was formerly known as Central Pentecostal College, and is located in Saskatoon, SK.
Please review the following information before proceeding to the Transcript Request Form below:
Transcripts cannot be processed and sent until payment is received. Transcripts are only available when student accounts are paid in full. For questions regarding transcript requests, please email kregier@horizon.edu
Students with Populi accounts can access their unofficial transcript and can request an official copy through their own Populi account.
In the Transcript Request Form:
- “Recipient” is where you want the transcript to be sent (for example: a university, church office, or your personal address, etc.)
- Submit 1 form for each Recipient (for example, if you want an official copy sent to a university as well as your own official copy, you will submit 2 forms).
- We do not need your SIN, please skip that field.
- Under “Additional Instructions” please include any relevant details such as:
- If you would like more than one transcript sent to the same Recipient
- Maiden name if applicable
- For official transcripts, payment will be made by credit card through the request form.
Cost
- Official mailed or emailed transcripts: $10 per transcript
- Unofficial copy by fax: $1 charge per fax
- Unofficial copy by email: no fee
- Seminary official copy to a school we have an MOU with (Briercrest or Providence): no cost
